Do you have concerns about your child's development? Visit the Oregon Screening Project. This a free screening tool for families to check on their child's development.

Our intake and evaluation process will look at your child’s development and whether special education supports will build skills to further your child’s individual developmental progress. Early Intervention serves children from birth to age 3; Early Childhood Special Education serves children from ages 3 and until they are eligible to attend Kindergarten.

Evaluations and screenings are free; if a child is found to be eligible for Early Intervention or Early Childhood Special Education, services are also provided at no cost to families. 

Screening and evaluations look at:

  • Adaptive skills: "self-help" skills -- such as getting dressed, putting on shoes or coat, eating, brushing teeth
  • Cognitive skills: brain-based skills -- such as understanding of colors, early numbers, matching/sorting, same/different
  • Expressive and Receptive Communication: expressive language is how a child communicates - using words, gestures, and sign language; receptive language is how they understand language - such as directions, gestures, etc.
  • Speech Articulation: pronunciation of sounds in words, parents/teachers/peers understanding what your child says
  • Motor skills: your child's ability to pinch or grasp objects (fine motor) and your child's ability to climb, walk, run, jump, etc. (gross motor)
  • Social skills: identifying self and others, sharing toys, participating in group activities, interacting with other children and adults

Early Intervention (EI) services are offered to eligible children from birth to age 3; Early Intervention supports each individual family in developing the skills to help their children learn and grow. Services are based on individual needs and are delivered through family support in each family’s home, other caregiving settings, or appointments at our service center locations; services can potentially include speech, physical, and occupational therapy.

Early Childhood Special Education (ECSE) services are offered to eligible children 3 to 5 years of age; ECSE services include specially designed instruction & family support and potentially related services such as physical, speech, or occupational therapy. ECSE services are based on individual needs and provided at community preschools (public and private), child care facilities, at ECSE center sites, and in-home.  

If your child will be of age to attend kindergarten in September of 2025 and is referred for an evaluation after April 30th, 2025, NWRESD will collect screening information via the ASQ and ASQ:SE. Based on the screening information, NWRESD will determine if an immediate evaluation is required.  If not, the screening results will be forwarded to the home school district for evaluation consideration. 

Referral Form and Teacher Observation Survey Instructions

  • Parents or guardians should complete the "Parent or Guardian" form linked below.
  • Teachers and childcare providers should complete the "Teacher or Childcare Provider" form linked below.
    • If you are a teacher or childcare provider and have been asked for input about a child already referred or in services, please complete the form linked below in the "Teacher Survey" section.
  • Medical providers should complete the universal referral form for medical providers. This is a form from the Oregon Department of Education. This is linked below.

Once a child has been referred, Northwest Regional ESD's Early Intervention/Early Childhood Special Education team will determine whether an evaluation is needed. We are currently offering virtual and in-person evaluations.
